Enhancing Alzheimer’s/Dementia Education and Care in Forsyth County

What is Memory Connections?

With funding from the Administration for Community Living, the Senior Services Memory Connections project is designed to help expand and improve our community’s ability to care for and support people living with Alzheimer’s Disease and other forms dementia.

This program focuses on:

  • Identifying people living alone who may have dementia and connecting them with services
  • Educating communities that are at high risk of developing dementia
  • Providing education and support to people caring for a loved one with dementia
