Assessment Nurse

Living at Home Program | Part-Time | 25 Hours Weekly | $25.25 per Hour

Senior Services is seeking a compassionate and experienced Assessment Nurse to support older adults and adults with physical disabilities enrolled in our Living at Home Program. This role plays a critical part in helping individuals remain safely in their homes by completing medical assessments and collaborating with a multidisciplinary care team.

This is a part-time position (25 hours per week) with flexible scheduling and limited remote work opportunities, depending on program needs.

What You’ll Do

As an Assessment Nurse, you will focus on both clinical assessment and care coordination, ensuring participants receive appropriate, person-centered services.

Primary Responsibilities

  • Conduct annual and change-of-status medical assessments for CAP/DA participants in their homes
  • Gather and document input from participants, families, physicians, caregivers, and service providers
  • Collaborate with Case Managers to develop initial and updated service plans based on assessed needs
  • Participate in interdisciplinary team reviews to meet NC Medicaid and program requirements
  • Ongoing Care Coordination
  • Maintain required medical documentation related to physician follow-up, medications, and supplies
  • Coordinate with Home Health providers when services fall outside the CAP/DA waiver
  • Monitor and document changes in health or psychosocial status and communicate concerns appropriately
  • Participate in Living at Home staff meetings and required trainings

What We’re Looking For

Required

  • Current, active North Carolina nursing license (RN/LPN)
  • Strong assessment, documentation, and communication skills
  • Comfort working independently in the community and as part of a collaborative team
  • Basic competency with Microsoft Word and Excel
  • Sensitivity to and respect for diverse backgrounds and lifestyles

Preferred

  • Experience in geriatrics, home health, long-term care, or CAP/DA
  • Knowledge of community-based resources and services

Work Environment & Physical Requirements

  • Primarily sedentary work with some local driving
  • Requires repetitive motion, close visual acuity, and frequent communication
  • No substantial exposure to adverse environmental conditions

Home Care Aide (CNA)

Home Care Department | $14.88 per Hour | Part-Time & Full-Time Available – Please specify preference on application.

Senior Services is seeking compassionate and dependable Home Care Aides (CNAs) to provide in-home support to older adults and individuals with disabilities enrolled in our Living at Home Program. This role is essential in helping participants remain safe, independent, and supported in their own homes.

What You’ll Do

As a Home Care Aide, you will provide hands-on care and practical support based on each participant’s individualized Plan of Care.

Primary Responsibilities

  • Provide personal care and mobility support, including assistance with daily living activities, transfers, and ambulation
  • Support participants with light housekeeping, meal preparation, errands, and respite care as outlined in the Plan of Care
  • Monitor participant well-being and communicate changes or concerns promptly to the Home Care office
  • Maintain accurate documentation and submit required paperwork on schedule
  • Follow all safety, confidentiality, and care standards, including OSHA and HIPAA requirements

What We’re Looking For

Required

  • High school diploma or equivalent
  • Certified Nurse Aide (CNA) in good standing with the State of North Carolina
  • Ability to follow Plans of Care and complete required documentation
  • Strong interpersonal skills and sound judgment when working with older adults and individuals with disabilities

Preferred

  • Experience working with older adults

Physical & Job Requirements

  • Ability to assist with lifting, positioning, and transfers (full or partial support)
  • Ability to lift up to 20 pounds
  • Frequent standing, walking, bending, and reaching
  • Ability to safely operate transfer equipment when needed
  • Dependable transportation with proof of automobile insurance
