Social Worker Case Manager – no longer accepting applications
The Social Worker Case Manager serves as part of a multi-disciplinary care team with an assessment nurse to develop service plans and provide ongoing case management to the aging population and adults with physical limitations enrolled in the Community Alternatives Program for Disabled Adults (CAP/DA), a Medicaid waiver program, in Forsyth County. Provide ongoing monitoring, support, education and linkage/referral as indicated by program policy and funding requirements.
Starting salary $46,640 – salary commensurate with experience
Health Care Associate – PT – no longer accepting applications
The Health Care Associate provides personal care to participants at the Williams Adult Day Center.
Starting pay rate $13.65
